Transition Support Services (TSS)

Transition Support Services assists individuals 14 years and older transitioning from school to life in their community by planning and offering a wide range of ideas and opportunities e.g. developing job skills/job placements in the community; building support networks and relationships outside the school system; providing functional experiences, activities and learning opportunities to promote future independence; creating a concrete foundation to build on their success when they graduate from the school system.
Collaboration with individuals, families personal support networks and school personnel is an important component of transition planning.
